Jon has mentioned it several times. Katie Agresta was hired when Jon's voice had died in the eighties and they've both gone on record saying it's her who helped him keep his voice throughout the Keep The Faith tour. On the This House Is Not For Sale tour one of her students (Marie Jo I think her name is) had joined Bon Jovi on tour to help him keep his voice. She was there front row telling him how to sing and pose.

As for the lack of warming up; yes he's done that before a show. But starting a tour from nothing and then doing 2 hour back to back shows without any warming up shows surely damages the voice. It's like a soccer player playing the odd ball in the field and then joining the champions league. Jon's always sounded best roughly 20 shows in the tour and then hits a breaking point after roughly 60 shows. From the Crush tour all the way to the 2011 tour it's been like this. Jon had a vocal coach throughout his entire career and he's still abused his voice to no end. It often sounds great but it was rarely doing him any favours.
