Mr. Google was no help on 'black hole heart attack'; but says "bag of nails" refers to 1) a popular corruption of the ancient inn sign, “The Bachannals", referring to a festival of drunken revelry, or to someone who indulges in drunken partying or is noisy and riotous when intoxicated; or 2) confusion, topsy-turvy, pointing every which way like the points of nails in a bag
