I think he's referring to health. His vocal muscles are in good shape and they can withstand a 21 song a night tour. Not raw, swollen, and bleeding.
His upper range is gone. And at times, his middle rage seems like that's gone too but his breathing might be part of the issues. Unless he does that on purpose to get through certain songs.
There are songs that he can still perform decent... They are basically the songs that no one cares about like Lost Highway.
I'm always listening to live playlists and every so often I'll be shocked that a song is from 2017 forward.
If I can interview one person, it would be Jon's vocal coach. I would love to know what's going on there.

I actually don't know if his upper range is completely gone or not. Yeah, he sounds strained on the high notes, yeah, for the most part he basically shouts them, yeah, he's uncontrolled, yeah, he cracks at times, yeah, he falls flat at times. But he nails them sometimes as well - on some occasions he seems able to sing higher than on others, sometimes he struggles as soon as any remotely higher note comes up. Born To Be My Baby, It's My Life, Bed Of Roses, I'll Be There For You, Saturday Night, Blood On Blood, Wanted, Lay Your Hands etc. he's been pretty decent on (considering the circumstances - and Walleris, please don't bring up the 120% effort analogy again...) for the most part since late 2017, whereas songs like Have A Nice Day, Keep The Faith, In These Arms, Bad Medicine, Prayer and These Days sound more or less dreadful every time.
On the other hand, his middle range is a complete mess. One can say it might be a matter of breathing technique, but his voice audibly is just so damaged that it simply doesn't come out properly, reminding me of trying to sing during a terrible flu with a sore throat. Songs like Lost Highway, Whole Lot Of Leavin', Born To Follow, Who Says and Captain Crash are actually among the worst offenders instead of the more decent ones, as Jon has basically lost all the tone of his voice and constantly sounds hoarse, monotonous and airy.
As for your health analogy, I'm afraid you're at least partially wrong. His vocal chords/muscles/whatever are obviously NOT in good shape at all, it's clear as a day and comes through with every single bum note, crack and clipping of his voice. Be it nodules, scar tissue or whatever that's hammering with his pipes, they're still far from healthy.
On the other hand, I'm also starting to fear his lungs might be affected as well. He never used to be as horribly out of breath as he was pretty much throughout the whole THINFS tour. The constant skipping of words and timing issues, clearly connected to his breathing, indicate some sorts of pulmonary/respiratory issues for me. It may not necessarily be a disease, let alone a life-threatening one, but we have to remember that Jon's now been a smoker (albeit on somewhat of an on-off fashion) since at least the Fahrenheit era, which makes it 35 years.

Jesus Christ, his lungs must look like burnt BBQ steaks, and that, if anything, affects everything in your voice; power, range, tone, stamina, stability, pitch control and especially breathing (technique).
So yeah, hard to say anything.
