Default help with CV

ok anyone out there who knows how to create a formal C.V?
could you give me the form?
name
surname
address etc
then i want to say that i can use the pc very well. how can i say it? "exceptional usage of personal computer" or what?
also i want to say that i've been taking lessons in spanish language (and i can use it properly :P ) but i don't have a degree yet. what's the formal way of saying this?

over here you'd cite the computer programs you are proficient in - pretty much everybody knows how to use a computer. Think about what programs they might be using and list them first. Instead of listing excel word etc. just say proficient in MS Office.

As far as language skills - "fluent in spanish" is the term used over here.

It's the same here, name the programs you can use (word, access, corel,...) and write it under interests or abilities/skills.

that's what i'm gonna do with the computer programmes but before that i wanna say something similar with what i mentioned above...
by the way the cv must be in english that's why i'm asking about the formal way i must do it.

for example the proper way to say where i live is "home address:... " or what?
also how do we call the person who describes sport games on the radio? spokesman?

vasika exo na metafraso ena cv apo ta ellinika sta agglika kai na diorthoso kai merika pragmata. to kyrio provlima einai stin metafrasi kai pali .
o syntaktis (se efimerides, sites, klp) metafrazetai journalist?
