I think that the surgery option is the most viable. His voice isn't just shot, or he would sound like 2013-14 (or like today's mariah carey to give another example). Many tend to forget that BJ songs are really high and "screamy", from 2005 to 2011 he certainly improved his technique, but he used it to push even more on songs like always or STBI in which he just pulled chest voice like there's no tomorrow (and that were too high even when he recorded them, as usual). No wonder there was that kind of decline, but in that decline he lost power in his voice, it became much thinner, but it was still him. If u check out some 2014 runaway shows you could hear that the problem was with the power in his high range, he had no breathing/tempo issues like he has today, and his tone was still "intact". The clinging to the mic thing is a physical thing, what do u do when at your favourite karaoke u have to sing LOAP or stuff like that? You squeeze every inch of your body to push your voice up there.
Another example? listen to Steven Tyler, yes he screams at 70, but his voice is much weaker than 10-15 years ago, that's "natural" voice decline.

Hmmm... the Rise Up New York! It's My Life acoustic with Phil X on the solo, I don't like it.
I also find this Greg Mayo on guitar and backing vocals more entertaining.

The best part was when he plays drunk wine pong with his kid. I was there for that. The attempt at BEP was also great!
